    1095-C Notification

In December 2024, A Paperwork Reduction Act was signed into law that lifts the mandatory requirement for employers to provide 1095-C forms to employees. As a result, the district will not be issuing 1095-C forms to all employees this year. If you would still like to receive a 1095-C form, please request one by reaching out to Cortney Hightower at cortneyhightower@lcsd56g.com. Please note, all requests must be received by March 31, 2026. 1095-C forms will be provided within 30 days of receipt of your request.
